BACKGROUND: Children diagnosed with velocardiofacial syndrome (VCFS) suffer from various disabilities. Palatal abnormalities, as well as speech and language impairment, adversely affect a child's quality of life (QoL) and are some of the most distressing aspects for the parents of these children. OBJECTIVES: The present study aimed to explore the effect of palatoplasty on the health-related quality of life (HRQoL) and speech outcomes in children with VCFS. MATERIAL AND METHODS: The study recruited 20 patients (N = 20) with VCFS and connected speech, aged 3 years or older, having either undiagnosed submucous cleft palate (SMCP) or velopharyngeal insufficiency (VPI), and requiring primary cleft palate surgery or revision surgery. Speech assessment was conducted prior to palatoplasty and 6 months after the surgery. Intelligibility and hypernasality were evaluated using the Cleft Audit Protocol for Speech - Augmented (CAPS-A). The parent proxy-report form of the Pediatric Quality of Life Inventory (PedsQL™) was used to evaluate and compare the HRQoL of the VCFS patients before and after palatoplasty. RESULTS: Significant improvement in the HRQoL scores was achieved after the surgery across all domains (physical, emotional, social, and school functioning), especially in the emotional and social dimensions (p < 0.000). The post-operative speech assessment based on CAPS-A demonstrated improvement in speech intelligibility and hypernasality in the majority of patients. CONCLUSIONS: Given that children with VCFS face various medical and social problems, suitable palatal interventions are beneficial, improving both the speech ability and QoL of these children.

OBJECTIVE: To investigate the effect of a tooth whitening agent on surface roughness, surface hardness, and force delivery properties of polyethylene terephthalate glycol (PETG) thermoplastic retainer materials of two different thicknesses. METHODS: PETG sheets (1 mm and 1.5 mm) were thermoformed over a 30â€¯× 60â€¯× 10 mm3 rectangular stone model. Surface hardness, surface roughness, and results of a three-point bending test were evaluated before and after treatment (5 h daily for 14 days) with a 15% carbamide peroxide home bleaching agent. Data were analyzed using an independent sample t­test. RESULTS: Exposure of PETG specimens to the bleaching agent for 14 days significantly reduced surface hardness and increased surface roughness of samples of both thicknesses. The magnitude of force at different deflection points decreased significantly in the 1.5 mm thickness specimens treated with the bleaching agent, but no significant change was observed in the specimens with 1 mm thickness. CONCLUSION: Use of a home bleaching agent could have adverse effects on surface roughness and hardness of PETG materials leading to reduced survival time of thermoplastic orthodontic retainers.

OBJECTIVE: Different surgical techniques and management approaches have been introduced to manage the cleft palate (CP) and its complications, such as otitis media with effusion (OME) and auditory problems. The optimal method, as well as the ideal time for palatoplasty and ventilation tube insertion, are the subject of controversy in the literature. We aimed to evaluate The Effect of Intervelar Veloplasty under Magnification (Sommerlad's Technique) without Tympanostomy on Middle Ear Effusion in Cleft Palate Patients. METHODS: non-syndromic cleft palate patients from birth to 24 months who needed primary palatoplasty from April 2017 to 2019 were enrolled in this study. intravelar veloplasty (IVVP) surgery under magnification has been done by the same surgeon. Likewise, Otoscopy, Auditory Brainstem Response (ABR), and tympanometry were performed for all the patients before and six months after palatoplasty. RESULTS: Tympanograms were classified into two categories according to shape and middle ear pressure, and it was done in 42 children (84 ears). Type B curve was seen in 40 cases (80 ears) before surgery which reduced significantly (P < 0.005) to 12 cases in the left ear and 14 cases in the right ear after surgery. So, after surgery, 70 % of the tympanogram of left ears and 66.6 % of the tympanogram of Rt ears were in normal condition (type A tympanometry). ABR was done for 43 patients (86 ears) before surgery and six months after palatoplasty. Data were shown that 40 of the patients had mild to moderate hearing loss before surgery, which reduced significantly (P < 0.005) to 9 in the left ear and 11 in the right ear after palatoplasty. So, after surgery, 79 % of ABR of left ears and 73.8 % of ABR of right ears were in normal status (normal hearing threshold). CONCLUSIONS: Intervelar veloplasty under magnification (Sommerlad's technique) significantly improved the middle ear effusion without the need for tympanostomy tube insertion.

STATEMENT OF THE PROBLEM: Periodontitis is one of the most common bacterial infections of the oral cavity. It is important to find adjunctive methods for chemical treatment of periodontitis with less complications and proven therapeutic properties. PURPOSE: The aim of this study was to compare the effects of Calendula officinalis and Hypericum perforatum on antioxidant, anti-inflammatory and histopathologic indices of induced periodontitis in male rats. MATERIALS AND METHOD: In this experimental animal study, forty adult male Sprague-Dawley rats were randomly divided into 4 groups (n=10) and then experimental periodontitis was induced by 3-0 nylon non-absorbable ligature. Each group was treated for 10 days as follows: 1) H. perforatum hydroalcoholic extract, 1000 mg/kg/daily, orally; 2) C. officinalis hydroalcoholic extract, 1000 mg/kg/daily, orally; 3) a mix of the two plants, 1000 mg/kg/ daily, orally, 4) normal saline solution. At the end of study, blood sample were obtained via cardiocentesis, the rats were euthanized, and their maxillae were removed. The samples were analyzed for histopathological scores and total antioxidant capacity and IL-1ß were measured. RESULTS: Mixed hydroalchoholic extract of H. perforatum and C. officinalis decreased IL-1ß (4.3020±0.63), and increased the antioxidant parameter in comparison to the control group (3.1192±0.43) (p< 0.001). There were significant histopathological differences between the treatment groups and the control group. CONCLUSION: Mixed hydroalchoholic extract of H. perforatum and C. officinalis might be considered as an adjunctive treatment for periodontitis.

ABSTRACT Objective: To evaluate different concentrations of Galla chinensis extract (GCE) added to poly(methyl methacrylate) (PMMA), which is widely used for fabrication of removable orthodontic appliances, regarding the effectiveness of this herbal extract on antimicrobial effect and flexural strength of PMMA. Methods: Acrylic resin samples containing 0.4%, 0.8% and 1.6% GCE were prepared. Flexural strength was investigated via three-point flexural strength test for the 15 acrylic resin blocks of each concentration. Disk diffusion test was used to evaluate antibacterial effects of incorporating the same concentrations of GCE into acrylic resin. All these three groups were compared with the control group, with no added GCE, regarding flexural strength and antibacterial properties. Results: Comparison of flexural strength between the three study groups and the control group showed significant differences between the groups (P=0.018). However, there was no significant difference between the groups containing GCE. There were significant differences in antimicrobial activity between the four groups (P=0.026). Conclusion: Within the limitations of this study, it is suggested that incorporation of GCE into PMMA would be beneficial for antimicrobial activity and flexural strength of PMMA, but further studies on other physical properties and antimicrobial effects on other bacterial strain would be beneficial prior to clinical investigations.


RESUMO Objetivo: Avaliar se diferentes concentrações de extrato de Galla chinensis (EGC) adicionado ao polimetilmetacrilato (PMMA), que é amplamente utilizado para a fabricação de aparelhos ortodônticos removíveis, interferem no efeito antimicrobiano desse extrato e na resistência à flexão do PMMA. Métodos: Foram preparadas amostras de resina acrílica com concentrações de 0,4%, 0,8% e 1,6% de EGC. Para a avaliação da resistência à flexão, utilizou-se o teste de flexão em três pontos para as 15 amostras de resina em cada concentração. O teste de disco-difusão foi utilizado para avaliar os efeitos antibacterianos da incorporação das mesmas concentrações de EGC na resina acrílica. Esses três grupos foram comparados ao grupo controle, sem adição do EGC, em relação à resistência à flexão e quanto às propriedades antimicrobianas. Resultados: As comparações dos três grupos com o grupo controle mostraram diferenças significativas (p=0,018) para a resistência à flexão. Entretanto, não houve diferença significativa entre os grupos contendo EGC. Foram encontradas diferenças significativas na atividade antimicrobiana entre os quatro grupos (p=0,026). Conclusão: Dentro das limitações desse estudo, parece que a incorporação de EGC no PMMA seria benéfica para a atividade antimicrobiana e a resistência à flexão do PMMA. Porém, estudos adicionais sobre outras propriedades físicas e sobre os efeitos antimicrobianos contra diferentes cepas de bactérias seriam interessantes antes de se fazer pesquisas clínicas.

BACKGROUND: Smoking tobacco is a significant health problem for humankind. Cigarettes could affect people's life from socioeconomic and psychosomatic aspects. The oral cavity is the first orifice through which cigarette smoke enters the body. Thus, it is directly exposed to cigarettes and their harmful ingredients. This study aimed to determine the effects of smoking cessation on oral health-related quality of life (OHRQoL). METHODS: The subjects in the present observational study consisted of individuals visiting a specialized smoking cessation clinic in Tehran, Iran, to give up their smoking habit. After documentation of the subjects' demographic data, the questionnaire [Oral Health Impact Profile (OHIP-14)] was completed twice in three months (before giving up smoking and three months after initiating the program to quit smoking). Data analysis was performed using Sig. (2-tailed), paired t-test, and one-way analysis of variance (ANOVA) at a confidence interval (CI) of 95%. FINDINGS: Thirty-one subjects (29 men, 2 women) participated in this study. The mean age of the subjects was 37.03 ± 11.30 years. Although OHRQoL scores were increasing as to some parameters, including food tastes, anxiety, and a feeling of shame in the subjects after giving up smoking, it was not statistically significant (P > 0.050). On the other hand, the relationship between the quality of life (QoL) (before and after stopping smoking) and age was significant (P = 0.001 before quitting and P = 0.050 after quitting). CONCLUSION: For a better understanding of the relationship between quitting smoking and an improvement in OHRQoL, it is necessary to perform more extensive studies in this field. The present study was a pilot study, which shed some light on the relationships between these parameters.

Oral mucositis is a common inflammatory complication among patients with cancer. This study evaluated the histopathological, stereological, and antioxidant markers of 2% eucalyptus extract in induced oral mucositis in male golden hamsters. In this animal study, oral mucositis was induced in 30 male golden hamsters by 5-FU (60 mg/kg) on days 0, 5, and 10 of the study. The cheek pouch was scratched with a sterile needle once daily on days 3 and 4. On days 14-17, 2% eucalyptus hydroalcoholic extract gel and Calendula officinalis extract gel groups were treated and then compared with a non-treated control group. The histopathological and stereological scores and the pouch content of malondialdehyde, as well as the activities of glutathione and myeloperoxidase in the pouch tissue, were evaluated. Histopathologic scores of oral mucositis were lower in the eucalyptus gel group than those of the calendula and control groups (p<0.05). Also, a lower malondialdehyde level and higher myeloperoxidase and glutathione activities were detected in the eucalyptus group in comparison to the calendula and control groups (p<0.001). The thickness of the mucosa and submucosa increased in the eucalyptus group. The numerical density of the fibroblast and the volume density of the collagen significantly increased in the eucalyptus group. In conclusion, the use of eucalyptus hydroalcoholic extract may be associated with reduced intensity of oral mucositis, diminished concentration of malondialdehyde, increased activity of myeloperoxidase and glutathione, increased volume of mucosa and submucosa, increased fibroblast and collagen in the induced oral mucositis in golden hamsters undergoing 5-FU consumption.

Resumo A mucosite oral é uma complicação inflamatória comum em pacientes com câncer. Este estudo avaliou os marcadores histopatológicos, estereológicos e antioxidantes de Eucalyptus 2% na mucosite oral induzida em hamsters dourados machos. Neste estudo em animais, a mucosite oral foi induzida em 30 hamsters golden masculinos por 5-FU (60 mg / kg) nos dias 0, 5 e 10 do estudo. A bolsa da bochecha foi arranhada com uma agulha estéril uma vez ao dia nos dias 3 e 4. Nos dias 14 a 17, os grupos de gel de eucalipto a 2% e curativos à base de gel foram tratados e comparados com um grupo controle. Foram avaliados os escores histopatológicos e estereológicos e o conteúdo de malondialdeído na bolsa, bem como as atividades de glutationa e mieloperoxidase no tecido da bolsa. Os escores histopatológicos de mucosite foram menores no grupo de gel de eucalipto a 2% do que os do gel e do grupo controle (p <0,05). Além disso, um nível mais baixo de malondialdeído e maiores atividades de mieloperoxidase e glutationa foram detectadas no grupo tratado com eucalipto em comparação aos grupos à base de gel e controle (p <0,001). A espessura da mucosa e submucosa aumentou no grupo Eucalyptus. A densidade numérica do fibroblasto e a densidade do volume do colágeno aumentaram significativamente nos grupos tratados com eucalipto. Em conclusão, o uso do extrato hidroalcoólico de Eucalyptus pode estar associado a menor intensidade de mucosite oral, diminuição da concentração de malondialdeído, aumento da atividade de mieloperoxidase e glutationa, aumento do volume de mucosa e submucosa, aumento de fibroblastos e colágeno na mucosite oral induzida em hamsters dourados em consumo de 5 UF.

PURPOSE: Numerous methods have been developed for blepharoplasty, including carbon dioxide (CO2) laser-assisted blepharoplasty. Although the superiority of CO2 laser compared with the scalpel for blepharoplasty has been proposed, to the best of our knowledge, no study has compared the clinical outcomes of blepharoplasty with the CO2 laser alone versus the combined use of a scalpel and CO2 laser. PATIENTS AND METHODS: In the present randomized clinical trial, 21 healthy patients underwent bilateral upper eyelid blepharoplasty. For each patient, an initial skin incision was made using the CO2 laser (setting, continuous emission; ultrapulse mode; 3 W of power) on 1 side and a scalpel on the other side. The remaining blepharoplasty steps were conducted using the CO2 laser (setting, continuous emission; 9 W of power) in both groups. The patients were evaluated on postoperative days 1, 3, 7, 14, and 30 using the postoperative repair criteria, including edema and ecchymosis. The Manchester scar scale was used to evaluate the results at 60 days after surgery. RESULTS: Our comparison of the 2 methods showed no significant differences at 1 month after surgery using the evaluation criteria. The scar index was not significantly different after 60 days, despite lower scores in the scalpel group. CONCLUSIONS: In upper eyelid blepharoplasty, making an initial incision with a scalpel, followed by use of a laser provides advantages similar to those found by performing the entire procedure with a CO2 laser alone.

BACKGROUND: Various techniques have been used for cleft repair such as the straight-line closure, the rotation advancement technique and the anatomic subunit approach which are famous new approaches gained popularity. However, these methods have several advantages and disadvantages and sometimes are difficult to adopt. In this study, we described our novel technique, known as separated multiple Y-to-V-plasty, in treatment of several cases of unilateral cleft lip. METHODS: Plastic surgeons usually try to avoid straight closure of the wound, especially in areas where they need to stretch, move and enhance the length of the wound in some stages of the procedure. Since the lip is a dynamic and active structure and is constantly moving, the use of straight-line incision and closure in that area is in conflict with this basic concept. RESULTS: In our newly introduced technique, we avoided a straight-line closure along the skin and mucosa for the repair of the unilateral cleft lip. This issue is of utmost importance in cases with severe loss of lip height and discrepancy. CONCLUSION: To acquire a natural and balanced shape in unilateral cleft lip repair, we recommended the novel Y-to-V-plasty technique as an effective method for severe unilateral cleft lip with enormous discrepancy.
